Survey Question
I need to attach a question to a course and be able to report on the responses. They need to check one of: Yes, No, I already did it. I need to be able to report to Compliance how they responded.

I tried to create this, load it in my LMS and created a report to extract my response. The response field is blank so I didn't do something right.

If you'd like the LMS to track the quiz, I recommend choosing the Final Assessment quiz type. If you'd like a quiz that isn’t scored or tracked by your LMS/LRS, then you'll want to choose Knowledge Check. You can read more about each option in this article.
