Quiz Skipping Questions

Hi,

I created a 25 question exam that the user has to take and pass after completing a course. When I test the exam, it appears to be skipping questions. Sometimes it'll only give me 23 questions. Other times, I get the complete 25. I've tested in both IE and Chrome. Any ideas as to what I need to fix?
