Any suggestions about how I can capture authorisations?

Hi,

I would like to create a authorisation and attestation page that I can send out to my SMEs to capture and record their final approval for a course to go live.

We currently use attestations to record when someone has viewed a course, but ideally I'd like to create a form that captures their role/title, name, date and their approval.

I'd need this information to be reported back to the the LMS so we could report against it for audit.

Has anyone got any suggestions about how I might do this Rise and Articulate?
