How do you create to separate assessment surveys within the same storyline?

Can you create an assessment for two results within the same storyline.  I'm trying to create an assessment that would ask two separate questions to the same categories that would give me a score or rating at the end of the assessment once completed by the participant. with out having to create to separate assessments . Initially each slide would pose a category with two questions the participant would respond to.
